Yes, it's called fraud. When you sign your driver's license, you are swearing that the information you are giving is accurate. I'm pretty sure that at the bottom of your license application just above the line where you sign it says that you swear that all information provided on the app. is true under penalty of law. Check it out.
Yes, providing false information to a governmental agency for purpose of defrauding them (i.e.- getting a license) is considered a felony.
